  5. Disconnect Between Frontline and Leadership — Employees feel unheard. Leadership feels unsupported. Communication gaps grow into cultural issues and turnover.
  6. No Process for Training or Onboarding — New hires are thrown into the fire. There’s no standard work or training structure. Consistency suffers — and so does morale.
  7. Accountability Without Follow-Through — Expectations are set, but there’s limited coaching, follow-up, or performance management system to reinforce them.
